




Job Summary: We are seeking a professional to manage the comprehensive follow-up of pediatric patients undergoing growth hormone treatment, ensuring access to and continuity of therapeutic care. Your role is to ensure access, authorization, and continuity of treatment, acting as a liaison between families and social health insurance providers to eliminate administrative barriers and guarantee therapeutic adherence. YOUR MAIN CHALLENGES WILL BE: What will your challenges be? Proactively contact parents or legal guardians of new patients to initiate support throughout the treatment access process. Inform and guide patients’ families regarding documentation required for medication authorization. Assist in organizing and submitting files to social health insurance providers and funders. Conduct continuous case follow-up until effective authorization is granted. Monitor file status and manage complaints in cases of delays. Early identification of administrative or regulatory barriers hindering medication access. Interact with social health insurance providers and other stakeholders in the healthcare system to expedite processes. Escalate critical situations to management when required. Conduct periodic follow-up of patients once medication administration has begun. Verify treatment continuity and detect potential interruptions. Identify early signs of low adherence and activate preventive actions. Provide emotional support and clear information to families facing questions or difficulties. Record every interaction and case progress in the systems defined by the program. Maintain up-to-date and organized information enabling full traceability for each patient. Prepare basic follow-up reports when requested by program coordination. · Requirements Advanced student or graduate of a Bachelor’s degree in Social Work. (Mandatory) Desirable experience in Patient Support Programs or case management in healthcare. Experience interacting with social health insurance providers and the healthcare system (desirable).
