A major healthcare institution is seeking two Pharmacists.
Schedule:
Night shift on Mondays, Wednesdays, Fridays, and alternate Saturdays from 20:00 to 08:00.
Main Responsibilities:
Carry out drug supply and magistral preparation activities in the laboratory according to HUA policies and procedures and current legal regulations, aiming to provide high-quality, safe patient care.
Validate medical prescriptions: verify maximum dosages, drug interactions, dosing frequency, adverse effects, and dosage adjustments based on patient condition.
Perform medication reconciliation.
Conduct clinical/pharmacological monitoring of patients considering their clinical/laboratory values.
Monitor drug shortages per patient and record relevant incidents.
Monitor financial resources allocated to patients under each pharmacist’s responsibility.
Collaborate in controlling substances causing physical and/or psychological dependence.
Report potential drug shortages and propose equivalent therapeutic alternatives.
Inform Nursing and Medical Staff about possible drug shortages and/or substitutions.
Conduct bibliographic research to support information dissemination, develop research papers/seminars, and contribute to continuous training of the pharmacy team.
Collaborate in the development project of a clinically oriented Hospital Pharmacy.
Provide ongoing training to Nursing staff regarding drug administration.
Preparation of magistral formulations (syrups, capsules, creams).
Preparation of drugs in laminar flow hoods (oncologic/non-oncologic). ON-CALL-
Assist in maintaining order and cleanliness within the department.
Specific Knowledge:
Knowledge of policies, standards, and procedures for adult ICU patient care (isolation protocols, hand hygiene), administration and monitoring of high-risk medications—including antibiotics, antifungals, antiretrovirals, vasopressors—and other agents contributing to hemodynamic balance in critically ill patients.
Patient transfer and mobilization.
Safe procedures, hospital techniques and procedures.
Requirements:
Licensed Pharmacist with provincial registration available for blocking (must not be a DT at another pharmacy).
Minimum 2 years of experience in similar positions.
